erm. the jewelry looks like a tunnel and a barbell, as to how the barbell goes through, probably self made holes....the piercing, no idea.

That is custom made jewelry. It's a stretching of the lobe with a barbell piercing the bottom of the conch through the stretch (through the entire lobe vertically). If there's a name to suit both piercings as one, I don't know what it is.
